HER2-RADiCAL is a UK trial that uses the response to treatment before surgery to decide how much HER2-targeted treatment people with early HER2-positive breast cancer need afterwards.   • Lunch with OPTIMA: Trial Results (16 June 2026, hosted by the OPTIMA trial team)

    • First results of the OPTIMA phase III non-inferiority trial, which randomised 4,429 people with high clinical risk, ER-positive, HER2-negative early breast cancer between standard chemotherapy followed by endocrine therapy and a treatment decision directed by the Prosigna gene-expression test. Patients whose tumours had a risk-of-recurrence score of 60 or below received endocrine therapy alone. Five-year invasive breast cancer-free survival was 91.5% in the control arm and 90.4% in the test-directed arm (hazard ratio 0.99), meeting the non-inferiority margin. SBSG members Iain MacPherson and Peter Hall led the study in Scotland.
